The crash occurred on the Port Fairy-Hamilton Road at the Kangertong Road intersection near Broadwater around 7.40pm on Wednesday.
Sergeant Mick Wolfe of Port Fairy police said the Macarthur man was driving north in his Holden Commodore when he veered into the south-bound lane where a milk tanker was slowing to turn.
"The tanker was only doing about 5km/h or 10km/h and the Commodore has veered on to the wrong side of the ride and collided head on into the milk tanker," Sergeant Wolfe said.
He said the 50-year-old received "severe injuries" and was flown by helicopter to the Alfred Hospital in a serious but stable condition.
The tanker driver was uninjured but left "a bit shaken and shocked", Sergeant Wolfe said.
He said it was possible fatigue was a factor in the accident as conditions were good and the road had a slight bend at the scene of the crash, but police were yet to interview the Macarthur man.
The Commodore received extensive damage and the driver had to be cut from the wreckage by the Port Fairy SES crew.
