The collision happened at a level crossing near Reilys Road, Woodvale, 13 kilometres north-west of Bendigo, about 2.45pm on Friday, a V/Line spokeswoman said.
The train was derailed and came to a stop near Quinns Road, where about 90 passengers were taken off the 12.50pm Swan Hill to Melbourne service, the Bendigo Advertiser reported.
Country Fire Authority crews attended to a fuel leak in the train, which was quickly contained, a CFA spokesman said.
The truck driver, aged in his 60s, was taken to Bendigo Hospital with a cut to his head, Ambulance Victoria spokesman John Mullen said.
A female train passenger was also taken to the hospital with minor injuries, while another passenger was treated for minor injuries at the scene, he said.
The V/Line spokeswoman said passengers were being put on coaches to continue their journey to Melbourne. All train services on the Swan Hill line have been suspended.
There were no cattle on the truck when it collided with the train, the CFA spokesman said.
