Portland police Sergeant Darren Hanegraaf said the accident happened on the bridge near the intersection of Canal Court and Cliff Street at 11.35am.
He said the woman driver was trapped in her car, before being transported to Portland Base Hospital and was being assessed.
Sergeant Hannegraaf said medical staff were deciding if the woman would be flown to Melbourne for further treatment.
