The three cases are linked to the current cluster in Corangamite Shire, and are all self-isolating, South West Healthcare said on Monday.
There are now 15 active cases across four south-west municipalities: zero active cases in Warrnambool, six active cases in Moyne Shire across three households, six active cases in Corangamite Shire across two households and three active cases across two households in the Glenelg Shire.
The positive COVID-19 case identified in the Portland region on October 23 has been found to have contracted COVID-19 outside the region, Portland District Health said on Monday afternoon.
"The positive case has taken all necessary steps to isolate and there are no local exposure sites as a result," PHD said in a statement.
The two cases who tested positive in Portland and returned to the Greater Melbourne are not included in the Glenelg Shire total.

South-west Victorian vaccination rates continue to climb
South West Coast MP Roma Britnell said it was "brilliant" to see all three municipalities in her electorate above 95 per cent first dose and above 80 per cent fully vaccinated as of today.
"I couldn't be prouder of my community's response," Ms Britnell said.
The region is likely to hit 90 per cent double dose by next Monday, and 95 per cent by the following week.
Federal vaccination data released Monday October 25 shows all south-west local government areas have passed the 95 per cent single dose vaccination target.
In Warrnambool 86.8 per cent of the population over 15 are fully vaccinated against COVID-19, or 28,859 members of the community.
In Moyne Shire 83.3 per cent are double dosed (13,498) and in Corangamite Shire 83.5 per cent (13,141) are fully vaccinated.
Glenelg Shire 80.1 per cent are double dosed (16,561) Colac Otway 79 per cent (17,696) and Southern Grampians 83.5 per cent (13,285).
Vaccination rates continue to climb among the region's Indigenous communities, but double dose numbers are still almost 24 per cent lower than the broader population.
82.06 per cent of the Indigenous population in Warrnambool and the south-west have received at least one dose of a COVID-19 vaccination and 59.61 per cent of the population is double dosed.
Broken down, 1276 of Warrnambool and the south-west's 1555 registered Indigenous community members over 15 have had one dose, and 927 are fully vaccinated.
It comes as the state government on Monday announced mobile vaccination vans operated by the Victorian Aboriginal Community Controlled Health Organisation (VACCHO) are being deployed to Warrnambool and the Grampians next week.
No new local exposure sites listed overnight
There were no new exposure sites listed in the region overnight.
Yesterday there were 64 tests completed across the Camperdown Leura Oval and Warrnambool testing clinics.
The COVID-19 testing clinic at Merri Street Warrnambool is open until 4pm today by appointment only.
The Leura Oval drive through testing clinic at Camperdown is closed today. The clinic will be open Tuesday, Wednesday and Friday from 3:30pm - 4:30pm.
"If in doubt, please get tested," SWH said.
"Thank you for doing the right thing to keep our community safe and, for your ongoing support."
Visitor restrictions at hospital to remain
Despite restrictions easing and vaccination milestones being reached locally, SWH said some visitor restrictions will remain in place for the hospital.
As of 4pm on October 24 there will still only be two visitors allowed per patient, per day, and one at a time.
Only visitors with a permitted reason as determined by the patient and family will be able to visit.
COVID-19 screening and masks will be required for all visitors and anyone with symptoms or visitors who have been in a COVID active area will not be permitted.
Visitors to Merindah Lodge are encouraged to be fully vaccinated.
