The division one and division three scratch teams take on Warrnambool.
Portland's Ben Cumming, a former golf professional, said the division one group was stoked to be in the decider.
"To finish on top of the ladder is a big achievement for our little club playing against Warrnambool and Port Fairy," he said.
Portland ended the home-and-away fixture with four wins and one loss - that being a 4-3 defeat to Warrnambool.

But they were missing Cumming and Julian Gollasch that day, who are both available Sunday.
Cumming is expecting another close encounter with the reigning champions.
Portland's team for Sunday is Cumming, captain Clint Foster, Daniel Forbes, Ken Banks, Andrew Sing, Gollasch and Phil Holmberg.
Cumming explained Barry Wilson was unavailable this week and Banks had come in to replace him.
Warrnambool has captain Blair Prescott, David Walker, Paul Walker, Caleb Perry, Alistair Gillin, Luke Williams and Mark Norton in its line up.
Not only are brothers David and Paul Walker playing in division one, Prescott pointed out Paul's son, Lachie, was playing in the division three scratch team.
While there was no men's pennant last year due to the coronavirus pandemic, Warrnambool has been the dominant team of recent years.
Prescott is going for his third pennant victory in a row as captain after 2018 and 2019 triumphs.
But he knows his side is in for a huge test against Portland.
Prescott said Portland had scored a convincing win against his team in recent seasons.
"We'll definitely have to be on our game for sure," he said.
The Warrnambool captain said the group had one loss this season to Port Fairy.
"That brought us back down to earth a little bit," he said.
Division one tee-off is 10.45am.
