The side, which is coached by former Allansford mentor Jason Saunders, led from start to finish to claim a 10.11 (71) to 3.4 (22) victory over Tyrendarra.
It's the first time the senior side has claimed back-to-back premierships during its dominant last decade.
The Lions have played in seven of the past eight SWDFNL deciders but have only secured four premiership victories.
The only grand final the club has missed since 2012 was in 2016. Before the '18-19 double it had secured premierships in 2012 and 2015.
Tyrendarra was also runner-up to Heywood in 2015.
Heywood burst out of the blocks in the opening term, slotting two goals to nothing.
In the second quarter both sides slotted a goal each but in the third term the Lions turned up another gear, banging home four goals to one to lead by 37 points at the final break.
Heywood added another three goals in the final term to run out 49-point winners.
Dakin Marris was named best on ground. He also kicked one of the Lions' 10 majors.

Dane Handreck, Gordon Slade and Ben Millard all slotted two goals while Will Lovett, Jarod Donovan and Leigh Malseed also hit the scoreboard.
Rob Gordon, Aaron Johnstone, Jason Walker, Millard and Will Bell were also named in the Lions' best.
Robert Alexander, Simon Marra and Mark Pitt were Tyrendarra's goal-scorers while Jack Cocks, Glenn Meulendyks, Todd Richardson, Tye O'Connell, Dylan Jennings and Beau Bentley were named in the best.
The Lions finished the regular season in fourth before defeating Dartmoor in an elimination final, last year's grand finalist Heathmere in a sudden-death semi-final and edged out minor premiers Westerns in a preliminary final to claim a spot in the grand final.
Tyrendarra, which finished second at season's end, defeated Heathmere and Westerns to secure its spot in Saturday's decider.
