An Ambulance Victoria spokeswoman said paramedics attended a Jancourt East farming property on Lavers Hill-Cobden Road about 12.20pm on Monday.
A four-year-old boy was air-lifted to the Royal Children's Hospital with a head injury caused by machinery.
Cobden police were called to the rural property and told the boy was standing on a cow milking machine when the incident occured just before midday.
WorkSafe were called to attend while Cobden police are investigating the circumstances of the accident.
A man identifying himself as the boy's brother wrote on The Standard's Facebook page saying "doctors are positive he will be ok".
