Lyndoch Living is a finalist in the improving consumer choice and increasing care and productivity categories of the innovAGENGING awards, while chief executive Doreen Power is a finalist in the age services innovator category.
Lyndoch Living board president Sue Cassidy said it was an honour for Lyndoch Living to be nominated.
She said to be named a finalist in three categories was fantastic.
"In the current environment, workforce and models of care are in the spotlight," Ms Cassidy said.
"It is exciting for Lyndoch to be able to highlight our social model of care and to be recognised for innovative thinking," Ms Cassidy said.
Ms Power said there was a move away from the traditional task-orientated model and a focus on really listening to the consumers, taking their life experiences and knowledge into account, and including their families in their care and service choices.
"The team has actively embraced this way of thinking," she said.
"We have turned the concept of ageing into the idea of living well and living healthy for our consumers that enhances their experience through individual choice, as one size does not fit all."
Head of innovAGEING Merlin Kong said the awards acknowledge the hard work the sector has achieved and how innovative it is.
"When we look at growing older in the best way - with the various government reforms at play, the technology available to us at the moment and changing service demands and productivity pressures - innovation is critically important," Mr Kong said.
"Innovation touches on everything that the sector is going through, as we work to spread excellence and transformation, to cement Australia as a world leader in better ageing."
The innovAGEING National Awards announcing the winners will be held virtually on November 26.
