The Seagulls won 33-32 in the grand final at Reid Oval.
It was a tight encounter with Seagulls taking one-goal leads to both half-time and three-quarter time.
Port Fairy co-coach Emily Williams, who led the team with Stacey Dwyer, said she was thrilled with the win.
"It was a fantastic game, we've had some ripping games with Koroit throughout the years," she said.
"Last year we were in the position where we lost by a goal to them (in the grand final) so that drove a lot our dedication this year.
"And the girls have been amazing and they deserve this win so much."
Koroit won by one-goal in a thrilling 2018 grand final.
