A 56-year-old Bannockburn woman is the third person to die after two cars collided on the Hamilton Highway on Friday afternoon.
Police said at the time that a white Hyundai wagon travelling on the Hamilton Highway collided with a maroon Mitsubishi about 3.15pm.
The occupants of the Mitsubishi, a 73 year old couple from Lindfield East NSW, died at the scene.
The female driver of the Hyundai sustained life threatening injuries and was airlifted to hospital in a critical condition, where she died on Tuesday.
Her 25-year-old Bannockburn rear seat passenger was also airlifted to hospital with serious injuries.
She remains in a serious but stable condition.
A 9-month-old baby boy, who was in a child restraint in the back seat of the vehicle, was uninjured.
It brings the state's death toll to 130, up significantly from the 85 lives lost by this time last year.
Warrnambool Acting Senior Sergeant Dean Greenwood said the incident was "tragic".
"It was a terrible scene, to have one death is awful, two is obviously tragic but three is just unheard of," he said.
"Detectives from the Major Collision Unit are investigating this tragic collision, it's still early in the investigation as it hasn't even been a week yet.
"They will do a thorough investigation and will be preparing a report for the coroner."
The Acting Senior Sergeant said it's a timely reminder to motorists to take further caution on the road in the lead up to winter.
"It's a timely reminder to get your vehicle in for a service, get your local mechanic to check your tyres and make sure your headlights are in working order," he said.
"Myself and my fellow members find it incredible frustrating that we can only do so much, we can't be everywhere all the time, people have to take responsibility for their own driving and their own actions.
"We don't want to be the ones to knock on your day and say your loved one is not coming home."
