Portland police Sergeant Daniel Lehmann said the accident happened at 5.30am on the Portland-Nelson Road about 500 metres south of Westlakes Road.
He said the driver was heading south in a white 1998 Holden Commodore sedan when he failed to negotiate a bend and crashed through a farm fence .
"The vehicle ended up about 100 metres off the road. The driver sustained minor injuries and Commodore received moderate damage," he said.
"The driver has a medical condition and fatigue could have been a factor in the accident. Police officers and an ambulance attended the accident scene."
Sergeant Lehmann said the farmer fixed the damaged fence to stop his stock wandering.
