The man in his 40s was a pillion passenger on a quad bike when he fell and hit his head.
The woman was riding the quad bike when the accident happpened about 8.30am last Tuesday.
The pillion passenger fell, hit his head and was knocked unconscious for about 10 minutes.
He was assessed and treated by ambulance officers and then placed in an induced coma before being flown by helicopter to The Alfred Hospital in Melbourne.
The man remained in the coma to allow him to best overcome his injuries and has been listed as being in a critical but stable condtiion.
It's expected he would remain in the coma for about five days.
Police may allege the woman rider refused to undertake an alcohol breath test after the accident.
Because she refused any court hearing the charge will presume she had a reading of .24.
The woman aged in her 40s appeared in the Warrnambool Magistrates Court less than two months ago on two drink driving-related charges.
She now has two prior court appearances for drink driving - one for more than three-and-a-half-times the alcohol limit and a previous offences for almost five times the limit.
After a previous single-vehicle accident she also refused an alcohol test.
In early May she was banned from driving for two years and placed on a two-year community corrections order with the conditions she do 80 hours of community work and undertake assessment, treatment and rehabilitation for alcohol abuse and use.
She also had to complete a road trauma awareness course.
Magistrate Ann McGarvie said when sentencing it was fortunate the woman had not killed anyone or herself - yet.
"But if you continue the way you are going you will and you will have to live with that for the rest of your life," she warned.
"Clearly you have a drinking problem and should not be driving," she said.
The Nirranda accident happened on a farm and the quad bike was registered.
WorkSafe officers are also investigating the accident.
