Dwyer, 29, tore three tendons off the bone during the Borough’s practice match win over Northern Blues on Saturday.
He is expected to miss between 14 and 16 weeks.
The classy midfielder-forward returned to Port Melbourne in the off-season after three years at AFL club Collingwood.
Dwyer, who started his football career at South Warrnambool, played eight seasons at the Borough before the Magpies threw him a chance as a mature-age rookie.
He went on to play 39 games for Collingwood before being delisted at the end of the 2015 season.
