The Geelong-based middle-distance runner will carry the Australian flag at the opening ceremony of the INAS Global Games in Ecuador on Tuesday, AEST.
Semmens will contest the 400 metres, 800 metres and relay, having bounced back to full fitness after an injury-interrupted two years.
He is one of three competitors with ties to the south-west competing. Warrnambool swimmers Caitlyn Mackay and Ashleigh Pettigrew are representing Australia in the pool.
The Games are organised by the International Sports Federation for Persons with Intellectual Disability and go ahead every four years.
