The 63-year-old man was expected to face the Melbourne Magistrate Court on Sunday charged with 58 counts of aggravated rape and associated offences.
His lawyer, Jessie Smith, said he had been taken to a Melbourne hospital after losing consciousness for an unknown reason at about 9.30am.
The man had been living in the United Kingdom, where he was surrendered to Australian authorities by local police on May 22 in response to an extradition request.
The case was expected to be adjourned until Friday.
