The former Magarey Medallist accepted the role on Tuesday night.
Crane joined Portland for the 2014 season, playing a key role in its midfield.
He replaces Jake Myles in the coaching hot seat.
Myles remains undecided about his playing future.
Portland president Troy Bannam said the club was thrilled to sign a player of Crane's calibre as coach.
"He is a very approachable guy who makes everyone feel welcome," he said.
"He is the perfect fit for what we wanted."
Cobden is the only Hampden league club yet to appoint a coach for next season after Port Fairy unveiled former South Rovers mentor Brett Evans as its new leader last week.
