Portland police said the driver was the sole occupant of his car, which crashed on the Woolsthorpe-Heywood Road just after 7am, about five kilometres east of Heywood.
The driver was freed from the vehicle by the SES.
CFA, ambulance and police also attended the crash site.
The man, believed to be aged in his 30s or 40s, was taken by air ambulance in a serious but not life-threatening condition.
Traffic was disrupted while the helicopter landed on Woolsthorpe-Heywood Road, but police said the road was completely cleared for traffic by 9.30am.
Police are investigating the cause of the crash.
