The collision occurred at the intersection of the Hamilton-Chatsworth Road and the Blackwood-Dunkeld Road at Woodhouse.
Hamilton Police located the scene just after 12.30pm.
It is believed a truck with a dog trailer travelling east collided with a sedan travelling north towards Dunkeld.
The truck was engulfed in flames and the driver died at the scene.
Four occupants of the sedan also died at the scene.
A woman who was trapped in the sedan was airlifted to hospital in Melbourne and later died.
A witness at the scene described it as "horrible".
"The car was just totally mangled," the woman said.
Major collision investigation detectives are now at the scene and surrounding roads have been blocked in all directions.
The intersection, known to locals and "the five ways" was once a notorious black spot with five roads all converging at one point. It has been modified and off-set in more recent years in an attempt to make it safer.
There was increased traffic in the area today with thousands of people converging on Dunkeld for the annual cup meeting.
